Harshaw 8801n with 3.6 mil Element 3.  These are standard harshaw TLD cards with three TLD-700 dosimeters and E-4 of TLD-600 material for neutron monitoring.  These have a thinner than usual E-3 (other cards are 6 mil) to allow for enhanced extremity monitoring.
